| Proof of name change (if any) such as a marriage license, court order, or divorce decree, etc. The document will be returned with the passport.) |
Proof of applicant’s departure date from USA, such as:
- Copy of applicant’s actual airline or cruise ticket; or
- Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
- A letter (on company letterhead) giving applicant’s name, date of departure, and the country to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.
